Here’s a few helpful pointers before you apply.

Check any deductible and confirm your claim is eligible


Note that some plans have a ‘deductible’ which is an amount you’ll need to pay before we pay the rest. Again, you may wish to get in touch with our team and check if this is the case.


Ensure you have no outstanding fees

Before we organise any cashless medical treatment, we’ll need to check there are no outstanding monies owed to FWD. For this reason, we advise you settle any outstanding bill(s) in advance.

The first step is to get a hospitalisation referral from your attending physician. Note that this plan covers cashless medical treatment at any private hospital in Hong Kong.

  • Please complete the form and return it to the Union Concordia Medical Group (UCMG) at least four working days prior to any hospital admission. UCMG is our pre-admission approval service provider.
  • Part II A and B to be completed by your attending doctor.

UCMG will process your application and get back to you within 2-3 working days.

If your application is approved, UCMG will call to inform you of the details. They will then send you an "In-Patient Credit Arrangement Authorisation" and "UCMG confirmation" (by email) within three working days from the date of approval. If you don’t receive these, you can contact UCMG on 2710 8105.

In the event your application isn’t approved, UCMG will advise you and offer more details. You’re welcome to discuss this further with us.

Upon admission

  • Please bring your "In-Patient Credit Arrangement Authorisation" and "UCMG confirmation".
  • Print and complete part I of the following form and pass it to the hospital. The hospital will arrange for your attending physician to complete part II of the form and submit to us.
Hospitalisation / Accident Claim Form

Note that upon discharge from hospital, you may have to pay an annual deductible amount. You’ll receive a claims statement after the claim has been processed. If there’s any shortfall, FWD will deduct this from your registered credit card.
